How We Measure Outcomes

Our goal is to help participants improve their quality of life and manage IBD with greater confidence. We aim for 8 out of 20 participants to improve their IBD disease management by achieving at least three of these four changes:

  • Reduce emergency room or hospital visits by at least 10%
  • Report increased confidence navigating the healthcare system
  • Increase social engagement
  • Implement dietary changes at home

  • Prepare for healthcare visits and strengthen communication with their care team
  • Identify healthcare and community resources
  • Navigate financial and insurance-related barriers
  • Understand available patient assistance resources
  • Build confidence in managing their care
  • Connect with additional educational and support resources

Our Navigators serve as a bridge between patients, healthcare professionals, and community resources, helping individuals take a more active role in their care.

Who We Serve

The program supports individuals diagnosed with Crohn's Disease or Ulcerative Colitis, with priority given to individuals and communities experiencing barriers to quality healthcare, education, and resources.

Our navigation services complement the care provided by a participant's healthcare team and do not replace medical care or medical advice.
